I acknowledge that I have received, read, and will comply with the Studio Policies. I will not hold New Vibe Dance Studio or any of its agents, employees, or representatives liable for any illnesses contracted or injuries sustained by me/ my child while a student at the studio, or participating in any class. I give permission for New Vibe Dance Studio to use photographs and/or video images of me/my child for advertising and marketing purposes.
