MHLV Dance, LLC, a Houston limited liability company (“the School”) will make all efforts to help our students to avoid injury. However, it is important that you understand and agree that in participating in any dance class, workshop, rehearsal or performance, there is a possibility of physical injury.
PLEASE READ CAREFULLY AND SIGN BELOW. REGISTRATION IS INCOMPLETE WITHOUT SIGNATURE AND MUST BE COMPLETED BEFORE YOUR FIRST CLASS I hereby acknowledge and agree to be bound by all the terms set forth herein in this Liability Waiver and Acknowledgement of Risk.
I voluntarily agree to assume all risks and responsibility for any injury or accident, which might occur to me or my child during any of the School classes, rehearsals, performances, or activities. I also exempt, release, and indemnify the School, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by the School.
I further voluntarily agree to waive my rights and that of my heirs and assigns to hold the School, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students liable for such damage, loss, injury, or death. I understand that I should be aware of my physical limitations and agree not to exceed them. If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights. I further hereby voluntarily agree that, for no additional consideration, the School shall have the right to use photographs or videos of me (or my child) for publicity purposes.
My signature is proof of my intention to execute a complete and unconditional waiver all liability, either on behalf of my minor child or on my own behalf, to the fullest extent of the law. I have read, understood and agree to be bound by the above statements:

MHLV Dance Student Information
Registration Fee: Yearly registration is $15. This fee is non-refundable and covers enrollment for a dance year (Spring, Summer, Fall).
Tuition and Payments: Tuition for the semester is due the first week of January. If paying by 4 installments, payment is due by the 10th of each month.
After the 10th, it is considered late and a $10 late fee may be posted to your account for that month. We do not offer refunds or prorates for missed classes (we only prorate classes on your first month of joining if you have missed any classes at the beginning of that month), however you can take any of our other classes as a makeup as long as it is within the same month. If you choose to leave the program at any point before your fourth class, you are entitled to a prorated refund, minus the registration fee.
If you choose to leave the program at any point after your fourth class, your future payments will be stopped, but no refund will be available.
We accept cash and check at the studio (please make all checks payable to: MHLV Dance), and automated online payments through our website (www.MHLVDANCE.com).

Spring Semester Rate
January 3 - June 10, 2021 $300 for 1 hour classes and $275 for 45 minute classes.
Can be paid in four installments, $70 or $60 per month

Makeups: If you miss a class due to illness, vacation, etc. we do allow students to take a makeup class. Makeup classes must be used within the same month as the missed class. If the studio closes for any reason out of our control (severe weather, disaster, etc.) classes will be rescheduled for a different date.
Class Arrival: Students should arrive 5 minutes before class and begin to put their shoes on and belongings away. 5 minutes early is on time. On time is late. We will always begin class at the scheduled time. Please be sure to pick them up no later than 10 minutes after class is over.
Class Etiquette
1. All students are asked to be polite, courteous and respectful to all instructors, fellow students, staff and parents.
2. All students must wear appropriate dance attire to each class (see list of dance attire on separate page). No sweaters allowed after warm up.
3. All students hair MUST be worn in a bun or pony (if too short, securely pulled back).
4. Dance shoes are to be worn inside only, students must not wear them outside, especially in the parking lot.
5. NO FOOD is allowed in ANY part of the studio.
6. Chewing gum is not permitted in class.
7. Jewelry is not permitted in class except small stud earrings.
8. Please do not interrupt your child's class, or open the door while class is in session. Parents may either stay in the studio the entire class or come in to the studio only 5 minutes before class ends.
9. Please be as punctual as possible, we do not want any students missing valuable class time.
10. If your child will not be attending that day, please notify the staff by email or phone.
11.. Please be sure your child uses the restroom BEFORE class begins. We do have a bathroom in the studio, but we do not want time wasted with bathroom breaks.
12. No cell phone use is allowed during class time.
13. Do not talk while the teacher is talking, be attentive and listen. Raise your hand if you have a question. Do not offer your opinion unless asked.
14. Please do not sit during class unless the teacher instructs you to.
15. No hanging or sitting on the barres.
16. You must ask the teacher's permission before leaving the classroom at any time.
Class Attire for ballet or jazz 1st -5th grade
Our uniform is strongly suggested. Wearing the proper attire not only makes you look and feel like a dancer but also allows the instructor to see your form and positions best.
Pink footed ballet tights, black leggings or jazz pants • Black Leotard or shell • Ballet Shoes • Ballet Skirt (optional, not mandatory) • Hair in bun or pony • Dance Bag/Backpack (to keep shoes in for class)
Children's dance attire (girls only) can be purchased at Target, Houston Skate & Dance Shop (5314 Bellaire Blvd) or if you prefer to purchase online Amazon.com and Discount Dance Supply (www.discountdance.com)
Hip Hop Classes Attire: Comfortable top, pants/leggings, sneakers
Teen/Adult Class Attire: Form fitting top or leotard, leggings or tights,. Jazz, ballet or lyrical shoes.
