All students must sign this waiver of claims and liability. By signing this waiver of claims and liability the student, parent or guardian acknowledges that they have read this agreement and will abide by all rules of INSTRUCTORS AND AMAYA DANCE. Signing this waiver also means that the undersigned/student releases Amaya Dance and its owners and instructors from any liability in case of injury or death from or related, directly or indirectly, to dance instruction at the Amaya Dance Studio on 1987 W TC Jester BLVD in Houston, Texas.

In consideration of being allowed to attend the Studio the undersigned/student acknowledges that learning various dances are physical activities and that participation may result in serious injury including death, and or other damage, due not only to the undersigned/students actions, inaction or negligence, but also to that of others associated with and or present at these or other classes. Furthermore, I acknowledge that there may be risks not known to me and or not reasonably foreseeable at this time. I personally, totally and completely assume all risks involved or in any way related to my participation in these classes or private instructions, whether known or unknown to me at the present time. Participation in these activities is voluntary and entirely of my own free will. I hereby represent and certify that I am in good physical condition, and have no diseases, injuries or other physical or mental conditions that would impair my ability to participate.

The undersigned/student, personal representatives heirs, executors and successors hereby waive any claim of accidental and or negligent tort damages or injury against Amaya Dance, its owners, invitees and instructors at 1987 W TC Jester BLVD
Houston TX, 77008 to include adjacent parking areas and driveways. The undersigned/student also certifies that they are covered by a medical insurance plan and when asked will present the needed information for approval to attend and participate at Amaya Dance.
