By agreeing to these terms & conditions you, the client, hereby understand and acknowledge that the training, programs, and events held by 365 Fitness & MMA may expose me to many inherent risks, including accidents, injury, illness, or even death. I assume all risk of injuries associated with participation including, but not limited to, falls, contact with other participants, the effects of the weather, including high heat and/or humidity, and all other such risks being known and appreciated by me.
I hereby acknowledge my responsibility in communicating any physical and psychological concerns that might conflict with participation in activity. I acknowledge that I am physically fit and mentally capable of performing the physical activity I choose to participate in.
After having read this waiver and knowing these facts, and in consideration of acceptance of my participation and 365 Fitness & MMA furnishing services to me, I agree, for myself and anyone entitled to act on my behalf, to HOLD HARMLESS, WAIVE AND RELEASE 365 Fitness & MMA, its officers, agents, employees, organizers, representatives, and successors from any responsibility, liabilities, demands, or claims of any kind arising out of my participation in 365 Fitness & MMA training, programs, and/or events.
365 Fitness & MMA recommends all clients to seek the advice of a physician prior to engaging in any physical activity in our studio. By signing this document, you confirm you have been cleared for vigorous physical activity by a physician.
I give 365 Fitness & MMA the right to display pictures, video, or any media with myself present in any way. If I decide to opt out of this, I agree to submit a letter of exclusion in writing to 365 Fitness & MMA. By my signature, I indicate that I have read and understand this Waiver of Liability.
I am aware that this is a waiver and a release of liability and I voluntarily agree to its terms.

All memberships obtained with 365 Fitness & MMA are a legally obligated to a minimum four-month contract before eligibility for contract termination. Continuous month to month payment plan and will continue until the member terminates the agreement in written email to 365fitnessmma@gmail.com after full commitment has ended.
Once month-to-month membership is active, you, the member, must give at least one months’ notice to terminate your membership. Written notice must be submitted by 11:59 PM on the last day of the month to make the following 1st your last payment; or member will be subjected to a pro-rated fee and/or another monthly charge.
Automatic billing takes place on the 1st of every month. If you prefer to pay with cash or check, you must still provide a card number to remain on file, and payment must be received prior to billing. If your account goes delinquent past one month, the membership will automatically be cancelled and subjected to a $150 penalty fee and $25 for each additional month not completed on agreed contract. All services are non-refundable.
